What is a California Advance Health Care Directive (AHCD)?

An advanced health care directive allows you to do either or both of the following things to prepare for the event that you become incapacitated:

• Appoint a Health Care Agent: You may appoint a health care agent who will have the legal authority to make health care decisions for you in the event you become incapacitated and can no longer speak for yourself.
• Provide Instructions for Health Care: An advanced health care directive allows you to provide specific written instructions for your future health care in the event you become incapacitated and can no longer speak for yourself. Your AHCD can spell out how you would like to be treated and any circumstances when you would like life support measures to be taken.
